Please remember you are representing LSI each time you work. Once you accept a work assignment we require that you do your best to please the client company to which you are assigned. In doing so, you will assure yourself of future assignments or an opportunity for permanent employment. Most of our jobs are full day assignments and may require more than 8 hours to complete. If you are unable to work more than 8 hours, you must notify us before accepting the assignment. Anyone failing to complete an assignment will be subject to termination. When the current assignment ends it is mandatory, and your full responsibility, to contact, communicate, "check in" and make yourself available for your next assignment. Failure to do so is considered a voluntary quit and ends the employment relationship.
When assigning you to a job LSI will share as much detail about the assignment as possible, including all pertinent and relevant informa- tion such as required skills, work environment, etc. There may be times that you will be asked to perform duties other than those we have told you. If at any time you feel that the job is dangerous or not within reason, call LSI immediately. If you are injured on the job, it is your responsibility to call LSI immediately. In the case of accepting a job assignment and not showing up for work without prior notification to LSI, I understand my failure to work is considered a resignation of employment and immediately terminates my employment.
I understand that LSI is my employer and is responsible for providing me with workers compensation coverage, overtime pay (as required by law) and all other responsibilities associated with being my employer of record. LSI requires all employees, other than those placed immediately into a permanent position, to complete a minimum of 500 hours with a client company prior to being eligible for permanent employment/hire.
It is the purpose of LSI to provide a safe, drug-free environment/workforce for our clients and employees. LSI has established the following drug testing policy.
LSI explicitly prohibits:
The use, possession or solicitation for sale of narcotics or other illegal drugs, alcohol or prescription medication without a prescription on company or customer/client premises.
Being impaired or under the influence of legal or illegal drugs or alcohol on company or customer premises.
LSI will drug test for the following reasons: pre-employment, random, post-accident and for cause. All employees are subject to drug testing prior to assignment. A random selection of employees will be chosen for testing conducted without prior notice. For cause drug testing will be performed when it is LSI’s belief that a drug problem exist. Examples: Evidence of drugs, alcohol, accidents/injuries in the workplace, fights or other incidents or behavioral symptoms of drug abuse, negative performance patterns, excessive absenteeism or tardiness. Employees who test positive or admit to substance abuse will be subject to termination. Employees involved in work related accidents/incidents will be tested. Testing positive for a post-accident drug screen will result in termination and the employee acceptance of the healthcare costs and medical bills for treatment following the initial visit.
On assignments requiring screening and testing, whereas the employee leaves the assignment prior to completion or test positive or fails a screening process while on assignment, the employee accepts and will be required to reimburse LSI for all associated costs.
I understand information and test results (example: drug screen, background check, MVR, other assessments) are used for the purpose of making employment decisions. LSI will treat all information as confidential, however as an employee I release that information to client companies for whom I would be assigned to work or interview for employment.
I hereby declare that all statements contained in this application are true and correct and understand that omissions or false or inaccurate information in the application will be the basis for termination. I hereby authorize LSI to investigate by background and verify this information. I understand that if employed, my employment will not be for any fixed period of time and may be terminated by the company or me at any time, with or without reason or notice. I agree to accept assignments for which I am qualified as they become available. Again, I understand my failure to report to LSI for work will indicate I have voluntarily quit. I also understand my require- ment to submit for drug, alcohol and other screenings during and prior to hire. Once hired, I may be required to submit to a medical exam or physical.
